Community stabilization services (REACH) for individuals with developmental disabilities

REACH logo and phone numberCrisis stabilization and related services, collectively known as “REACH” (Regional, Education, Assessment, Crisis Services, Habilitation) are available 24/7 for individuals of all ages with documented evidence of intellectual and/or developmental disability and individuals over 18 years of age with mental health and/or at risk behavioral challenges (qualifications described further below) who live in areas served by the following Community Services Boards (CSBs): Alexandria, Arlington, Fairfax-Falls Church, Loudoun, and Prince William.

REACH services include mobile crisis response teams for crisis stabilization and prevention for adults and youth as well as crisis therapeutic homes for both adults and youth in need of short-term crisis stabilization and prevention outside of their home (30 days or less for crisis stays and 5 days or less for prevention stays).

Eligibility

  • REACH works with people who live in areas served by the following Community Services Boards (CSBs): Alexandria, Arlington, Fairfax-Falls Church, Loudoun, and Prince William, including:
    • Individuals of all ages with an intellectual and/or developmental disability (IDD) who may also have a co-occurring mental illness and/or significant behavioral challenges.

Intensive Community Support

  • Support provided in the individual’s current residence.

Adult and Youth Crisis Therapeutic Homes (ATH/CTH)

  • Residential crisis stabilization for up to 30 (adult) or 15 (youth) days.
  • Step down transition from hospitals, jail, and training centers.
  • Prevention stays to add additional support during periods of high stress.
  • Serves up to six adults (ATH and CTH) and six youth (CTH).
  • Provides clinical and behavioral support on a time-limited basis (up to 30 days, typically shorter for youth) to stabilize the individual and assess ongoing needs.

Other Services

  • 24/7 mobile crisis support – respond to crises in the community within one hour.
  • 24/7 crisis line.
  • 24/7 face to face response.
  • Assess and determine support needs.
  • Develop crisis or behavioral plans (cross-system planning).
  • Referral services for ongoing treatment (community/prevention services, emergency medical, CSB intake, etc.).

REACH Mission

  • Individuals with intellectual and/or developmental disabilities shall be supported with services that allow the individual to live the most inclusive life possible in his/her community which includes access to appropriate and effective crisis stabilization, intervention, and prevention services including mental health treatment services when indicated.

REACH Purpose

  • To provide meaningful and exceptional services so that children, adults and families living with disabilities and mental health challenges can live, learn, work and play in their communities.
